OHSU Health Questions Survey
September, 2008
Dear Friends;
We have been invited to help with a
research study that is being done by the Research and Training
Center on Health and Wellness at
the Oregon Health & Science University (OHSU) in Portland, Oregon.
These Oregon researchers are studying
the kinds of questions that are used to find out about people’s
health. This survey will
help the researchers know what the best questions to use are to find
out about people’s health, even if they have disabilities.
We were chosen to send out their
survey to test the new questions.
The OHSU researchers will send you a $20 gift card if you
will answer all the questions in the survey and send it back to them
in the postage paid
envelope they will provide.
It’s that simple. (And don’t forget to complete their yellow
contact information sheet. Send it back with the completed survey so
that you can get your gift card!)
Only 700 completed surveys will be
used from all across the country.
Let’s make sure our state is represented.
The survey will take about 40 to 50 minutes to complete. It
has been approved by the University’s Institutional Review Board,
which means that your personal information is separated from the
answers to your questions.
None of your personal
information is made public.

Requirements:
Have a TBI for 6 months or longer, be over 18, and be able to read, understand and answer survey.
